Client Services Agreement

Radiant Nutrition provides individualized Medical Nutrition Therapy and nutrition counseling by licensed, registered dietitians in person at the Argyle office or via HIPAA-compliant telehealth.

Description of Services

The initial appointment is approximately two hours and includes a comprehensive nutrition assessment, goal-setting, and an initial care plan. Follow-up appointments are typically 30–60 minutes and focus on monitoring progress and adjusting the plan as needed.

Insurance & Payment

Radiant Nutrition is in-network with many major insurance plans. Any balance not covered by insurance — including co-pays, deductibles, and non-covered services — is the client's responsibility. Self-pay clients pay at the time of service. Major credit cards, HSA/FSA cards, cash, and check are accepted.

Cancellation & No-Show Policy

Radiant requests at least 24 hours' notice for cancellations or rescheduling. Less than 24-hour cancellation may incur a $50 fee. Missed appointments without notice may incur a $75 no-show fee.

Scope of Practice

Radiant registered dietitians provide evidence-based nutrition counseling and Medical Nutrition Therapy. Services are not a substitute for medical care, and Radiant does not diagnose medical conditions or prescribe medications.

Telehealth Services

Telehealth sessions are conducted through a HIPAA-compliant video platform. Clients receiving telehealth must be located in Texas at the time of service.
